I have a friend who knows how to limit google forms to one IP. I can ask him to show me how to do it. I like the idea of doing the sub-surveys and then posting the links to the sections on a wordpress or website. Also apparently now there is also google forms which will compile survey data for you. I've seen friends use it. It's similar to surveymonkey and the like.

Well, I for one am glad they didn't put me down when I failed a round of doxycycline. :) I was diagnosed with hypogammaglobulinemia 1.5 years into having Lyme untreated. There's a gap between 0.5 years and 1.5 years where they didn't take any bloodwork, so it could have started as early as 0.5 years...
