Yes, it can still progress while in treatment, but that usually means that the treatment is not adequate, or you got more than one kind of germ from the tickbite and something else needs to be added to the treatment. As usual, every time they say some manifestation of lyme is rare, I repeat that what is rare is when they diagnose it. No one at all knows how often this occurs. What is sure though is that it is more common than suspected.
